Supporting Your Community Health Needs Assessment*

As of 2012, IRS regulations require that all non-profit hospitals conduct a community health needs assessment and to adopt an implementation strategy to meet the needs assessed every three years in order to maintain their 501(c)(3) status.  Our highly qualified team can assist you and your stakeholders to design and develop a community health needs assessment and improvement plan that fulfills this government mandate and your community’s needs.

Our Experts Are Ready to Assist You

With specialized expertise in the fields of public health, qualitative and quantitative research, evaluation, and health improvement planning, our team is positioned to effectively and efficiently:

  • Manage the collection, analysis, and interpretation of health-related data;

  • Design and conduct focus groups, surveys, and key informant interviews;

  • Identify and assist in the prioritization of health needs within your community; and

  • Collaborate with key stakeholders to develop your community’s Health Improvement Plan.

*Our team works collaboratively with your organization to design and deliver health assessment, evaluation, and program services tailored to your specific needs and budget.  We welcome the opportunity to discuss your project details. The initial consultation is provided at no cost, and a proposal is then developed based on the project scope desired.

Accolades

When the TAHD became the fiduciary for the Litchfield County Community Transformation Grant (CTG), we needed an experienced professional that would meet our deliverables, at a reasonable fee, within the allotted timeframe. Mary was the perfect person for the job. Mary went above and beyond what she was hired to do, and helped lead both the CT Department of Public Health and the other four Connecticut CTG Counties in meeting the CDC requirements for our Community Health Needs Assessments. Mary is and will continue to be our “go-to” person.

Robert Rubbo, MPH, RS, Director of Health, Torrington Area Health District

“The Center for Healthy Schools & Communities @ EdAdvance was instrumental in the successful completion of the Community Report Card for Western CT and in facilitating the 2013-2014 Strategic Community Health Improvement Planning process in our region. The Center's highly skilled professionals worked very effectively with our community partners to capture their viewpoints and concerns about obtaining positive health outcomes in our communities. They collaborated well with our diverse partnership that includes Western CT Health Network, the United Way, the Regional YMCA, the City of Danbury Health Department, other health departments and districts, as well as service providers and stakeholders throughout the 10-town region.  The Center is a highly-valued and respected resource and we look forward to their involvement in our health assessment and improvement initiatives in the years ahead."

Scott T. Leroy, MS, MPH, Director, City of Danbury Health & Human Services

“Mary was essential to the successful completion of our last two countywide Community Health Assessments. She is extremely organized and has excellent writing and data analytical skills. Mary collaborated with our staff, hospital, and community partners with skill and efficiency to produce a wonderful resource.”
